Ongoing observationNo individual observes an entire decision system. Executives, engineers, regulators, researchers and practitioners each occupy different positions and therefore observe different phenomena.
The Institute compares those independent observations, identifies recurring patterns, and refines its understanding accordingly. Through comparison, challenge and accumulation, isolated observations become patterns. Patterns become evidence. Evidence strengthens, refines or refutes the discipline.
The purpose is not to collect opinions. The purpose is to improve collective judgement.
If you occupy a position that gives you sight of something others may not yet have recognised, a pattern in how authority moves, a structural vulnerability in decision systems, a recurring failure mode in AI governance. The Institute welcomes thoughtful contributions.

Field Note 001
Human in the Loop tells us that a person remains somewhere within the decision process. It does not tell us whether that person still exercises meaningful authority over the outcome. Those are different things.

03. Why Nothing Else Solves It
Decision Science
Studies how individuals and groups make better choices: cognitive quality, bias reduction, structured analysis. Optimises the decision. Not the architecture of authority within which decisions are made.
Corporate Governance
Establishes accountability at the ownership boundary of the corporation: boards, fiduciary duty, shareholder rights. Addresses who controls the firm. Not how authority flows through it.
Administrative Law
Disciplines the exercise of delegated authority by public institutions: transparency, participation, reasoned decision making. A legal framework for public bodies. Not an engineering discipline for private organisations.
Systems Thinking
Has modelled how authority should flow within organisations since Beer’s Viable System Model and Simon’s Administrative Behavior. Describes the structure of authority. Does not treat it as an engineering object.
None of these disciplines treat consequential organisational authority as an engineering object.
AI did not create the problem.
It revealed it.
